Semaglutide belongs to a class of medications known as glucagon-like peptide-1 (GLP-1) receptor agonists.
Tirzepatide is in a class of medications called glucose-dependent insulinotropic polypeptide (GIP) receptor and glucagon-like peptide-1 (GLP-1) receptor agonists
They are both once per week (on the same day each week) injections into the subcutaneous layer (fat layer) of the skin.
Semaglutide-
The Hormone GLP-1 after eating a meal begins a chain reaction by increasing insulin and lowering your hormone responsible for eating. This then causes your body to digest your food at a slower rate, in turn causing your body to tell your brain it's full.
Tirzepatide-
GLP-1 and GIP are peptide hormones that are secreted with the benefit of utilizing cells from intestines called enteroendocrine cells in reaction to the consumption of nutrients, and they have an important function in postprandial metabolism. With the identification of glucose equilibrium, their most favorable condition, the incretin effect, begins to improve the glucose-stimulated release of insulin from the pancreas. GIP is considered to be the essential incretin hormone accounting for this effect, although when administered together, they exhibit a synergetic effect.
Significantly less hungry, less “food noise” generalized fullness. I strongly recommend a B12 or B complex supplement which you can get from Walmart, Amazon or any store with a pharmacy section. This helps energy levels.
The medication is injected with a very small insulin sized syringe and needle, it isn’t painful!
No, this is not through insurance. These are generic forms of the medications known as Ozempic and Mounjaro - and getting them through insurance averages 1000-1800$/month for weight loss.
Compounding 503A/B pharmacies. There are a multitude of options but I work with 3 pharmacies exclusively because of their product and the purity of it. These certainly can be found cheaper, but that does not translate to better.
You will complete an intake form with medical history and discuss together after I review that information.
Additionally, I do require two lab tests (if you’ve had them in the last 6 months, I will accept them).
They are a CMP (comprehensive metabolic panel) and TSH (thyroid stimulating hormone). The reason for this is to assure that you do not have other factors like a low functioning thyroid, kidney disease or liver disease in addition to electrolyte abnormalities.
If you don’t have recent labs from your Primary Care Provider, for both tests, it would be $50 (not optional to be billed through insurance) at the Stephenson County Health Department with turnaround time under one week (let’s keep our money local!)
Preferably, you would abstain completely from alcohol with these medications.
That being said, although I don't recommend drinking, modesty is key.
You will notice you can’t drink more than a few without feeling unwell.
Tirzepatide is more expensive but boasts a 22-24% overall bodyweight loss versus 15-18% with Semaglutide.

Greasy and spicy, high fat and heavily processed as they will lead to general feeling of unwell.
Both medications start to work in a few hours after first injection.
Every 7 days, they last for a week. The medication is at its peak effect from day 3-5 so we can modify your start date with that in mind. For example, if weekends are a challenge, inject Wednesday.
Nausea, GERD, headache, lightheadedness (particularly on initial dosing or dosing increase). If you have a history of pancreatitis, your risk of this is higher to recur.
Constipation- this can happen but remember you’re eating much less so will have less bowel movements. If this is a concern, contact me and we will figure out together the best course of action for you.
*side effects are anecdotally lesser with Tirzepatide.
Notify your PCP right away if you get a lump in the throat or neck, with animal studies showing thyroid cancer of note- this hasn’t been found in human studies and was extremely rare in animals studied.
The most important is that you drink at least 1/2 gallon of water per day. The other “trick” is to inject in the back of the arm for lesser side effects. Anecdotally, it has also been recommended to drink a protein shake prior to or immediately after injection to lessen side effects.
I recommend half gallon per day, with good reason, it will decrease risk of nausea as a side effect, constipation, fatigue also lesser with minimum 80oz of water or non alcohol liquids per day.
Never go below 1200 calories per day. Your body isn’t made for that and will rebound poorly if you do this kind of “crash diet” approach.
Aim for fruits, vegetables, protein and stay away from greasy/spicy or processed, high fat foods as they’re going to increase risk of side effects.
Age 12-17 Obese children over 95% weight for sex and age *age 12-15 must be cleared by pediatrician in order to start medication
18+ BMI over 27, will not be prescribed for BMI under 22
Family or personal history of medullary thyroid cancinoma or multiple endocrine neoplasia syndrome type 2 (MEN-2)
Pregnancy, breast feeding or attempting conception
History of recent or recurrent pancreatitis
Will only increase your results, but be mindful of working out at high intensity for the first 4 days after starting or the first 2 days of a dose increase.
Drink your water, exercise, be mindful of your body and what it’s communicating.
Stop eating when you’re 75% full, don’t eat for 3 hours before bedtime to reduce risk of GERD side effect.

You could go on and off medication for weight loss, but I don't want that for you. I want you to understand your habits and your "why" to fix it. Obesity is a chronic condition, and therapy can help you with this. Do you eat to rid yourself of anxiety, depression, or other reasons? This would be addressed by a licensed therapist.
I highly recommend treating the whole body, which includes the mind, heart and soul.
